How Vitamin D Benefits In Health And Arthritis

The mechanism by which vitamin D gets absorbed in your body is very less understood. When we expose ourselves to the sun, the skin absorbs vitamin D and sends it to the liver.

If you take vitamin D via supplements or food, then the gut sends vitamin D to the liver. The liver then converts this vitamin D to a chemical.

This chemical gets distributed throughout the body where the different organs then turn it into an active and functional vitamin D. This form of vitamin D then performs its actions on the body.

One of the most important functions of vitamin D is to maintain the level of calcium absorption in the body to make the bones stronger and healthier.

What Does It Do

The glucosamine in your body helps keep up the health of your cartilage — the rubbery tissue that cushions bones at your joints. But as you get older, your levels of this compound begin to drop, which leads to the gradual breakdown of the joint.

There’s some evidence that glucosamine sulfate supplements help counteract this effect, although experts aren’t sure how.

Some people have also used glucosamine to try to treat rheumatoid arthritis and other conditions, such as inflammatory bowel disease, asthma, allergies, chronic venous insufficiency, sports injuries, temporomandibular joint problems , and long-term low back pain. So far, though, there’s not much scientific evidence that it works for those problems.

Can I Have Too Much Vitamin D

Taking more than 100 microgram of vitamin D as supplement a day can be harmful.

For most people, taking a 10 microgram vitamin D supplement every day will be enough.

Having too much vitamin D from a supplement over a long period of time, can lead to a build up of calcium in the body, a condition called hypercalcaemia. This can weaken bones and can be bad for the heart and kidneys.

If you have any concerns, you should consult a doctor.

Its not possible to get too much vitamin D from sunlight, but you should avoid the risk of getting sunburnt by covering up if you are out in the sunshine for long periods. Sunburn can increase the likelihood of developing skin cancer later in life.

It Prevents The Progression Of Arthritis

Cytokine-mediated pathways play a central role in the pathogenesis of rheumatoid arthritis.

This is because rheumatoid arthritis is an auto-immune disease and cytokines mediate an important role in regulating the adaptive and innate responses of the immune system.

It is important to regulate levels of cytokine for normal functioning of the immune system.

This can prevent the occurrence or progression of rheumatoid arthritis.

An experimental study evaluated the effects and mechanisms of Vitamin E on serum levels of cytokines.

The conclusion made was that Vitamin E reduced levels of cytokines in the serum suggesting that its supplementation in the diet can be used as a novel method in the treatment of rheumatoid arthritis.

What does this mean? Rheumatoid arthritis is an autoimmune disease and cytokines play an important role in regulating conditions of the immune system. Vitamin E in the diet can reduce high levels of serum which are responsible for the auto-immune conditions. This approach of incorporating vitamin E in the diet can be used as an effective treatment of arthritis.

Rcts Of Vitamin E For Inflammatory Arthritis

Serum vitamin E levels more than doubled over 12 weeks in the treatment group in a trial in RA, but Edmonds found no significant differences between groups in three clinical outcomes regarded by the authors as indicators of inflammation. There were significant differences favouring vitamin E in patient recorded morning pain, evening pain and pain after a chosen activity, but these analyses used one-tailed tests. A re-analysis confirmed significant differences favouring the vitamin E group for morning and evening pain but not for pain after a chosen activity. It is not clear how daily patient data for the pain scales were gathered and which of these data were used in the analysis at 12 weeks. Differences between groups in patient global assessments failed to reach significance while investigator assessments significantly favoured vitamin E. Even so, according to investigator global assessments, 60% of patients showed no change or got worse. Baseline data indicate that the vitamin E group had more severe RA than controls. The authors concluded that vitamin E has analgesic but not anti-inflammatory effects. The latter conclusion was supported by haematological and biochemical data gathered during the study.

The second RCT comparing vitamin E with placebo is that of Jäntti described earlier. The abstract available provides no assessable data but the authors concluded there was no clear effect of vitamin E on RA.

What vitamins are good for joint pain?

After an acclimation period, the rats were injected with collagen that was emulsified in CFA. Briefly, 5mg collagen from chicken sternum was dissolved in 2.5mL cold acetic acid . This mixture was emulsified with 2.5mL CFA and then homogenised for 15min. This solution was prepared on ice to ensure that the protein in the emulsion was not denatured. The rats were anaesthetised, and arthritis was induced by injecting 0.05mL collagen type II emulsified in CFA intradermally/subcutaneously into each paw of the hind limbs. Subsequently, two booster injections of the same concentration of ovalbumin were administered on day 7 and day 14 , as shown in supplementary information, Supplementary Figure .

Machtey compared vitamin E with placebo for 10 days in a single-blind, crossover RCT in patients with various OA diagnoses, mainly spondylosis. The only clinical outcome was a daily patient record of the condition categorized as âno painâ, âmarked improvementâ, âsome improvementâ, or âno changeâ. However, in the analysis, the data have been categorized as âsignificant improvementâ, âmild improvementâ or âno improvementâ but how these categories relate to the four originally recorded by the patients is not explained. Nor is it explained how the daily patient records were converted to a single overall category for each of the 10-day treatment reports. The authors report that 15 of 29 patients experienced âsignificant improvementâ during treatment with vitamin E compared with 1 of 25 during treatment with placebo, a statistically significant difference . Combining the categories, âsignificantâ and âmild improvementâ indicated that 22 patients benefited from vitamin E treatment and 13 from placebo treatment . It should be noted that no washout period was incorporated into the crossover design and assessors were not blinded to treatment allocation.

Wluka carried out a 2-yr RCT comparing vitamin E and placebo in OA of the knee. There were no statistically significant differences between treatment groups for loss of knee cartilage, a quality of life measure or in a disease specific, instrument completed by patients.

Trials For Rheumatoid Arthritis

Three of the four trials in this review article investigated the potential role of vitamin E in treating rheumatoid arthritis, while the fourth studied the effectiveness of selenium ACE treatment.

  • In the first trial , there was no significant difference between participants on 1,200 mg per day vitamin E supplements or a placebo in morning stiffness, number of swollen joints and degree of joint tenderness, but there was a significant difference in pain reduction in favour of participants taking vitamin E.
  • In both the second trial and the third trial , there was no significant difference between participants allocated 1,200 mg a day vitamin E supplements or participants given 150 mg diclofenac a day in all aspects of disease severity at the end of the trial.
  • In the fourth trial , there was no significant difference between participants who received selenium ACE supplements or participants on a placebo in all aspects of disease severity at the end of the trial.

A trial of low quality. Results of this trial were given a lower weighting when we came to our conclusion about the compound.

Collection And Analysis Of Blood Samples

At days 0, 21, 42, and 55 post-surgery, sera were obtained after blood centrifugation for 15 min at 2000 ×g to measure VE. Vitamin E was determined by high performance liquid chromatography with ultra-violet detector , using Hewlett Packard Series 1100 liquid chromatograph according to the method published elsewhere and modified in our laboratory. Briefly, samples were isocratically eluted with a mix mobile phase respectively of acetonitrile and methanol and detected at 285 nm wavelengths through a Hewlett-Packard 1046 fluorescence diodearray detector . Column type was Zorbax Eclipse plus C18, 3.0 x 50 mm, 600 bars . External and internal standards were prepared from -tocopherol and -tocopherol acetate and were injected before samples were taken. Concentrations of VE were determined in microgram/deciliter from a standard curve of the peak-area ratio of the analyte-internal standard, plotted against the concentration of analyte. Conditions of compliance criteria and quality control included: coefficient of determination of standard curve > 0.98 relative standard deviation 20% detection limit = 85% to 115%, and quantification limit = 80% to 120%. This method was also used to determine the VE concentration in synovial fluid after the dogs were euthanized . Standard hematological and biochemistry analyses were carried out on day 0 and day 56 by the Diagnostic Service of the Faculty of Veterinary Medicine at the University of Montreal.

A 5-ml venous blood sample was collected from the cubital vein of each patient at baseline and postintervention. All samples were centrifuged at 4000 rpm for 10 min and stored immediately at 80 °C until analysis. The synovial fluid was aspirated prior to opening of the knee. The synovial tissue was collected from the most inflamed and redness area of the anterior femoral fat pad during TKA. The synovial fluid was then instantly centrifuged to remove cells and joint debris. Similarly, both the synovial fluid and tissue specimens were stored at 80 °C until sample collection from the last patient had been completed. Multiple investigations of oxidative stress, vitamin E, and antioxidant capacity were performed as described below.
