How Does Vitamin D3 Work
Vitamin D is a fat-soluble vitamin that can be found in only a few foods, added to others, and obtained as a dietary supplement. It’s also made naturally by the body when sunlight hit the skin and stimulate vitamin D synthesis.
Vitamin D obtained from sun exposure, food, and supplements is biologically inert and must undergo two hydroxylations in the body for activation. The first occurs in the liver and converts vitamin D to 25-hydroxyvitamin D , also known as calcidiol.
Calcitriol circulates as a hormone in the blood and regulates calcium and phosphate metabolism for bone formation and other physiological functions. Vitamin D sufficiency is achieved when serum 25D concentrations are 30 ng/mL .
Most vitamin D is derived from sun exposure however, the amount of sun exposure required to maintain adequate levels varies by age, skin pigmentation, season, latitude, time of day, clothing habits, and use of sunscreen.
People who have limited sun exposure need to include good sources of vitamin D in their diet or take supplements to achieve and maintain adequate serum 25D concentrations.
Elevated Blood Calcium Levels
Vitamin D helps your body absorb calcium from the food you eat. In fact, this is one of its most important roles.
However, if your vitamin D intake is excessive, your blood calcium may reach levels that can cause unpleasant and potentially dangerous symptoms.
The symptoms of vitamin D toxicity are primarily related to hypercalcemia, which means excessively high blood calcium levels (
Hypercalcemia typically develops after people take megadoses of vitamin D for a prolonged period of time.
For example, a 2015 case study reported that an older man with dementia who received 50,000 IU of vitamin D daily for 6 months was repeatedly hospitalized with symptoms related to high calcium levels .
In the 2020 case report mentioned earlier, the woman who took an average of 130,000 IU of vitamin D per day for 20 months was also hospitalized for symptoms related to hypercalcemia .
These included nausea, vomiting, muscle weakness, and kidney injury. Her blood calcium levels were 3.23 mmol/L (
- poor appetite
However, not all people with hypercalcemia experience the exact same symptoms.
One woman experienced nausea and weight loss after taking a supplement that was later found to contain 78 times more vitamin D than stated on the label .
Importantly, these symptoms occurred in response to extremely high doses of vitamin D3, which led to calcium levels greater than 12 mg/dL.
Drug Interactions Of Vitamin D2 Vs D3
Vitamin D2 and D3 each are metabolized by the liver to 25D, and therefore the potential drug interactions are similar for both forms. Vitamin D may increase the serum levels of aluminum when taken with aluminum hydroxide, a common antacid, and therefore the combination should be avoided. Thiazide diuretics, such as hydrochlorothiazide, may increase the chance of vitamin D raising calcium levels in the blood to a dangerously high level. Patients on both thiazide diuretics and vitamin D supplementation should be monitored for this effect by their healthcare provider. Some drugs may decrease the absorption and effectiveness of your vitamin D supplement. Bile-acid sequestrants, such as cholestyramine, are an example of a drug which will impair vitamin D absorption. Vitamin D and cholestyramine should not be administered at the same time.

A: Research I have done in this area has found that people with low blood levels of vitamin D have a greater risk of a heart attack, heart failure, stroke, diabetes or high blood pressure later in life. In pregnant women, low vitamin D levels are linked to pre-eclampsia, gestational diabetes and adverse pregnancy outcomes. No matter your age or stage of life, having adequate vitamin D levels is important.
How Much Vitamin D3 Should A Person Take
There is no Recommended Dietary Allowance for vitamin D. The Institute of Medicine recommends a daily intake of 600 IU of vitamin D for most adults between the ages of 19 and 70 years, including pregnant and lactating women.
The IOM set an Upper Limit of 4000 IU per day for adults to avoid the potential risk of adverse health effects associated with excessive vitamin D intake.
Pregnant and lactating women, as well as people over the age of 70 years, are advised to get more than 600 IU/day because they are at increased risk for vitamin D deficiency.
People with dark skin are at increased risk for vitamin D deficiency and may need higher intakes of vitamin D. Some researchers recommend that people with dark skin should get up to 1000-2000 IU of vitamin D per day.
A number of factors can affect how much vitamin D you need, including your age, skin color, and level of sun exposure. Talk to your healthcare provider about how much vitamin D you should take.

Is Vitamin D Or D3 More Effective
Vitamin D2 and D3 are absorbed into the bloodstream where they are metabolized by the liver into 25-hydroxyvitamin D2 and 25-hydroxyvitamin D3, otherwise collectively known as 25D or calcifediol. Calcifediol is the vitamin D complex circulating in your blood, and its levels directly reflect your bodys stores of vitamin D. Calcifediol is commonly referred to as the active form of vitamin D. When your doctor orders lab tests to check your vitamin D levels, they are measuring your calcifediol levels.
There have been several studies comparing whether supplementation with vitamin D2 or D3 produces a higher blood level of calcifediol. A study published by the National Institutes of Health was conducted in elderly, post-menopausal women who had been identified as vitamin D deficient. It compared the effects of receiving a single high dose of vitamin D2 or vitamin D3 on calcifediol levels. The study concluded that vitamin D3 produced about twice the amount of circulating calcifediol in this patient population versus that of vitamin D2.
In a separate clinical trial comparing a 10-week regimen of twice weekly 50,000 IU dosing of both vitamin D2 and vitamin D3 in demographically matched groups, vitamin D3 was also found to be superior in producing higher levels of 25D, or calcifediol.
How Does Vitamin D Work
Vitamin D is a fat-soluble vitamin that is both a nutrient and acts as a hormone, explains Melissa Prest, D.C.N., R.D.N., spokesperson for the Academy of Nutrition and Dietetics. Its main function is to help the body absorb and regulate calcium and phosphorous, ensuring theyre available to strengthen bones and teeth, she adds. It also aids in the absorption of magnesium, which supports muscle and nerve function as well as energy levels.
Vitamin D boosts the immune system, too, by inhibiting the production of inflammatory cytokines, a molecule that signals inflammation, explains Heather Mangieri, R.D.N., a sports and wellness dietitian in Pittsburgh, Pennsylvania.
There are two main types of vitamin Dvitamin D2 and vitamin D3. Vitamin D2 naturally occurs in some plants, whereas vitamin D3 is found in some animals and produced by human skin via sunlight exposure. Mangieri explains that D3 is considered the more potent form of the vitamin because it has been shown to raise levels higher and for a longer period of time than D2.
