Vitamin D Side Effects
There are health risks for taking too much vitamin D, but perhaps not at the levels previously suspected. Taking a 50,000 IU âvitamin D supplementâ weekly is not likely to get you to that level. But it’s a good idea to know what these possible dangers are and at what intake and blood level the mainstream medical community says you are at risk.

Other Potential Negative Effects
But toxicity may not be the only thing to worry about. Even at lower intakes, as low as 30 to 48 ng/ml, the NIH reports potential risks including increased all-cause mortality, greater risk of some cancers, such as of the pancreas, increased risk of cardiovascular events, and a higher incidence of falls and fractures among older adults.

Conflicting Vitamin D Recommendations
Not everyone agrees with the National Academy’s proposed RDA and UL. Some reputable sources believe that people need well more than the RDA, and in fact more than the UL. For example, the Vitamin D Council, a California-based nonprofit, recommends adults take 5,000 IU daily or 8.3 times the RDA. People with overweight and obesity may require as much as up to 8,000 IU per day, or more than â13 times the RDAâ.
According to the Endocrine Society, the current official guidelines are based specifically on bone health but do not take into account vitamin D levels needed to prevent other conditions that may result from vitamin D deficiency. Specifically, NAM set the benchmark for deficiency at a blood level of 20 ng/ml of 25-hydroxyvitamin D, the main circulating form of vitamin D in the body. However, the Endocrine Society classifies blood levels below 29 ng/ml as insufficient and recommends a better target is 30 ng/ml or higher.
Director of the General Clinical Research Unit and Bone Health Care Clinic at Boston University Medical Center, Michael F. Holick, M.D., Ph.D., told Endocrine News, a publication of the Endocrine Society, that he believes blood levels between 40 and 60 ng/ml are an even better goal and that blood levels up to 100 ng/ml are perfectly safe.
Warnings Of Vitamin D And D3
Vitamin D toxicity may occur with excessively high doses. Symptoms could include nausea, vomiting, loss of appetite, constipation, dehydration, fatigue, and confusion. Due to the fact that vitamin D is a fat-soluble drug, the effects of vitamin D therapy can last 2 months or more after therapy is discontinued. It is also important to know the vitamin D content of other supplements you may be taking, such as a daily multivitamin. You should not take high doses of vitamin D supplements without your doctors instruction.

Are Vitamin D Supplements Necessary
If you have a vitamin D deficiency, taking a supplement may be beneficial.
Many people are at a higher risk of deficiency, including older adults, those with darker skin, people with certain health conditions, and individuals who dont regularly get sun exposure .
Additionally, vitamin D supplements may be recommended for those with health conditions that affect nutrient absorption, such as cystic fibrosis, inflammatory bowel disease, celiac disease, and short bowel syndrome .
If you suspect that you may have a vitamin D deficiency, its best to consult a healthcare professional to have your levels checked and determine whether a vitamin D supplement is necessary.

Are Vitamin D Or D3 The Same
When we refer to vitamin D, we are talking about Vitamin D2. Vitamin D2 and D3 are both widely used vitamin D supplements but are not the same. Vitamin D2 is ergocalciferol and comes from plant-based sources. Vitamin D3 is cholecalciferol and comes from animal-based sources. Both supplements are processed in the body by the liver to 25-hydroxyvitamin D, though vitamin D3 is thought to provide higher levels of 25D. Some formulations of vitamin D2 are prescription only, while all formulations of vitamin D3 are over-the-counter.

How Much You Need
The NAM developed the recommended daily allowance, or RDA, based on the needs of the general population. This is the amount it has determined will prevent deficiencies and the negative health effects caused by getting too little vitamin D in 97.5 percent of people.
The RDA for vitamin D for men and women, ages 19 to 70, is 600 IU daily. If you take 7,143 IU each day, that’s nearly 12 times or 1,200 percent of the RDA.

What Should You Do
These conflicting recommendations and data pose a dilemma for the consumer. A daily intake of 7,000 IU is highly unlikely to cause vitamin D toxicity and very unlikely to cause other problems. However, there’s no way to be sure.
If you think you may be deficient in vitamin D, see your doctor for a âblood testâ. Get her expert advice on the right blood level. If testing shows you are below this level, then follow your doctor’s recommendation for how much vitamin D to take each day.
How To Use Weekly
Take vitamin D by mouth as directed. Vitamin D is best absorbed when taken after a meal but may be taken with or without food. Alfacalcidol is usually taken with food. Follow all directions on the product package. If you have any questions, ask your doctor or pharmacist.
If your doctor has prescribed this medication, take as directed by your doctor. Your dosage is based on your medical condition, amount of sun exposure, diet, age, and response to treatment.
If you are using the liquid form of this medication, carefully measure the dose using a special measuring device/spoon. Do not use a household spoon because you may not get the correct dose.
If you are taking the chewable tablet or wafers, chew the medication thoroughly before swallowing. Do not swallow whole wafers.
If you are taking the rapidly-dissolving tablets, dry your hands before handling the medication. Place each dose on the tongue, allow it to dissolve completely, and then swallow it with saliva or water. You do not need to take this medication with water.
Certain medications can decrease the absorption of vitamin D. Take your doses of these medications as far as possible from your doses of vitamin D . It may be easiest to take vitamin D at bedtime if you are also taking these other medications. Ask your doctor or pharmacist how long you should wait between doses and for help finding a dosing schedule that will work with all your medications.

Is Vitamin D2 The Same As Vitamin D3
Vitamin D2 and D3 are slightly different. Both forms raise your blood vitamin D levels, and both can treat and prevent rickets from vitamin D deficiency. However, studies have found that vitamin D3 effectively raises blood vitamin D levels for a longer period than vitamin D2.
Additionally, vitamin D2 and D3 are derived from different sources. Vitamin D2 is plant-based. It is made by exposing ergosterol found in yeast to ultraviolet light. Vitamin D2 can also be found in some types of mushrooms. Vitamin D3 is sourced from sheep lanolin. It is made by exposing the lanolin to UV radiation to convert the cholesterol in lanolin to vitamin D3.
Vitamin D supplements can contain either vitamin D2 or D3. However, most over-the-counter preparations contain vitamin D3.

Does Vitamin D3 Give You Energy
Increasing vitamin D intake is thought to improve energy. A clinical trial was done examining fatigue levels in patients who were identified as vitamin D deficient. These patients were given vitamin D3 supplementation to increase blood levels of vitamin D, and the results showed a significant improvement in fatigue levels. Scientists have shown this may be due to vitamin Ds effect at the cellular level where it augments mitochondrial oxidative phosphorylation in skeletal muscle. This decreases muscle fatigue.

Surrogate Versus Clinical Outcomes
Although 25-hydroxyvitamin D levels are a better surrogate than a rat binding study, clinical outcomes are the most relevant. Low levels of 25-hydroxyvitamin D are very common in North America and have been associated with a wide variety of diseases including osteoporosis, bone fractures, malignancy, cardiovascular disease, infections, and more. The majority of the data supports association, not causation, of low vitamin D levels. In other words, much of the data does not clearly support the idea that vitamin D supplementation in a patient with low vitamin D levels reduces the risk of these diseases.
With that said, a recent Cochrane review analyzed the all-cause mortality benefit of vitamin D supplementation by pooling 95,286 participants from 56 randomized clinical trials. In this meta-analysis, about 80% of participants received cholecalciferol and about 20% received ergocalciferol . The vast majority were women , and most were older than 70 years of age.

Why Is Ergocalciferol Prescribed
If ergocalciferol is less potent and has a shorter duration of effect, why is it used? The most likely reason is due to dosage formulations. Unlike cholecalciferol, which is typically only available as a maximum dose of 5,000 units per capsule or tablet, ergocalciferol is available as a monster 50,000 unit dose. This larger dose may be seen as more convenient for patients or healthcare providers who want to provide supplementation on a weekly or monthly basis.

Common Side Effects Of Vitamin D Vs D3
There are no common side effects to therapy with either vitamin D2 or D3. Side effects related to vitamin D are the result of hypervitaminosis D, an extremely rare condition that occurs when you intake too much vitamin D. This is sometimes seen in patients who take megadoses of vitamin D, leading to vitamin D toxicity. The result is a build up of dangerously high levels of calcium in the blood which can lead to nausea, vomiting, constipation, and frequent urination. If left untreated, irreversible renal failure can occur along with calcification of organs and soft tissues.
The following table lists side effects related to hypervitaminosis D, not normal vitamin D supplementation. More information on vitamin D toxicity may be obtained from your doctor or pharmacist, as this may not be a complete list.

I took my first dose of 50,000 units of vitamin this past Friday night. I was prescribed 50,000 units once a week for 12 weeks.
I experience adrenalin rush symptoms along with a little shakiness and insomnia. However once I fell asleep, I stayed asleep until I woke up abruptly feeling like I couldnt catch my breath and my heart pounding I sat straight up in bed and violently coughed. It really scared me. After I calmed down all was fine and I was able to relax.
Has anyone had a similar experience? What can I expect after the 2nd dose? 3rd dose? etc.

Elevated Blood Calcium Levels
Vitamin D helps your body absorb calcium from the food you eat. In fact, this is one of its most important roles.
However, if your vitamin D intake is excessive, your blood calcium may reach levels that can cause unpleasant and potentially dangerous symptoms.
The symptoms of vitamin D toxicity are primarily related to hypercalcemia, which means excessively high blood calcium levels (
Hypercalcemia typically develops after people take megadoses of vitamin D for a prolonged period of time.
For example, a 2015 case study reported that an older man with dementia who received 50,000 IU of vitamin D daily for 6 months was repeatedly hospitalized with symptoms related to high calcium levels .
In the 2020 case report mentioned earlier, the woman who took an average of 130,000 IU of vitamin D per day for 20 months was also hospitalized for symptoms related to hypercalcemia .
These included nausea, vomiting, muscle weakness, and kidney injury. Her blood calcium levels were 3.23 mmol/L (
However, not all people with hypercalcemia experience the exact same symptoms.
One woman experienced nausea and weight loss after taking a supplement that was later found to contain 78 times more vitamin D than stated on the label .
Importantly, these symptoms occurred in response to extremely high doses of vitamin D3, which led to calcium levels greater than 12 mg/dL.
