As an adult over 40 I had suffered from a case of long term mononucleosis during most of 2011. A large proportion of my naturopathic patients present with some form of fatigue and fibromyalgia type pain. In many cases, the fatigue may be a symptom of a coexisting condition. For example, fatigue is one of the many symptoms of diseases such as rheumatoid arthritis, anemia, hypothyroidism, congestive heart failure, and so forth. In fact, fatigue can be a symptom of many disease processes. This is why a conventional workup, including history, examination, and when necessary, laboratory investigation, is necessary to rule out other disease processes that may contribute to fatigue.

In these cases, the fatigue is secondary, and will likely improve when treatment for the primary condition has been successful.

But what about the patient, where fatigue persists and no conventional cause or diagnosis can be found?

The numbers are actually quite staggering, as the CDC states that up to 4 million Americans suffer from the disease that has been labelled chronic fatigue syndrome. When specific types of pain and sensitivity are present, then the diagnosis is chronic fatigue syndrome and fibromyalgia. Diagnosis can be a challenge because there are no consistent lab biomarkers that are specific for CFS that are readily available to test, patient’s do not look sick, as mentioned above, fatigue is common to many other illnesses, and finally, chronic fatigue syndrome and fibromyalgia can tend to relapse and remit.

How Is Cfs Diagnosed

  • There is no specific diagnostic test for CFS. A diagnosis of CFS first requires that other potential causes of fatigue be ruled out.
  • Diagnostic criteria for CFS include chronic, unexplained fatigue for at least six months, which is of new onset, is not the result of ongoing exertion, is not substantially relieved by rest, and hinders occupational, social, or personal activities.
  • Two additional symptoms must also be present: unrefreshing sleep and post-exertional malaise .
  • Impaired memory or concentration and/or worsening of symptoms on standing up or sitting upright is also necessary for a diagnosis of CFS.

Study Sheds Light On Origins Of Diabetes

An international study unprecedented in both scale and scope has provided some important answers to the mysteries of how diabetes developsnot least of which is the answer to a century-old debate about whether genetic differences are shared and common, or rare and individual.

Led by researchers from the United States and England, more than 300 scientists from 22 countries used DNA from 120,000 individuals to pinpoint genes and their variants.

The collaboration brought together two research projects: GoT2D and T2D-DENES. Treatment Of Candida Infection

Candida albicans is a common resident of the healthy human intestinal tract. However, overgrowth of candida may cause health problems, including CFS-like symptoms, and may be involved in CFS in some individuals. A study found a higher number of C. albicans in fecal samples of CFS subjects during acute CFS symptom exacerbation compared with the remission phase when symptoms were less pronounced.151 A positive response of a large number of patients with CFS to an oral antifungal agent and a diet for intestinal candidiasis has also been described.152 The observational and uncontrolled nature of these studies make more rigorous study of the relationship between candida overgrowth and CFS necessary.

Mistake # 2 All Supplements Are Created Equal Brand Doesnt Matter

Because the FDA does not regulate the supplement industry, supplement companies release products without federal oversight and unknown quality control. Independent testing to ensure that each supplement has the ingredients listed on its label is not required. Dr. Craig only uses supplements that carry one of three labels on the bottle that indicate it has passed a test by an independent agent that ensures the facilities that produce the supplement are adequate and/or that their supplements are pure and potent.

Viral And Other Infections

Post-viral fatigue syndrome or post-viral syndrome describes a type of chronic fatigue syndrome that occurs following a viral infection. A recent review found Epstein-Barr virus antibody activity to be higher in patients with CFS, and that a subset of patients with CFS were likely to have increased EBV activity compared to controls. Viral infection is a significant risk factor for CFS, with one study finding 22% of people with EBV experience fatigue six months later, and 9% having strictly defined CFS. A systematic review found that fatigue severity was the main predictor of prognosis in CFS, and did not identify psychological factors linked to prognosis. One review found risk factors for developing CFS after mononucleosis, dengue fever including longer bed-rest during the illness, poorer pre-illness physical fitness, attributing symptoms to physical illness, belief that a long recovery time is needed, as well as pre-infection distress and fatigue.

Similar risk factors have also been found following the bacterial infection Q-fever. The same review found biological factors such as CD4 and CD8 activation and liver inflammation are predictors of sub-acute fatigue but not CFS however, these findings are not generally accepted due to the use of the Oxford criteria in selecting patients. The CDC does not recognize attribution of symptoms as a risk factor.
