A world-first clinical trial led by the Centre for Eye Research Australia has shown that vitamin B3 could play an important role in protecting against nerve cell damage that leads to blindness in glaucoma.

Results of the trial, led by Professor Jonathan Crowston and Dr Flora Hui at the Centre for Eye Research Australia, are published today in Clinical and Experimental Ophthalmology.

They show significant improvement in the visual function of glaucoma patients who received a daily high dose of 3 grams of nicotinamide for 12 weeks in addition to their regular treatment to reduce eye pressure.

Dr Flora Hui, from the Centre for Eye Research Australia, said a larger international trial was now needed to determine if the improvement shown in this study could be sustained over the longer term to reduce the progression of glaucoma.

The current study, which also includes researchers from the University of Melbourne, Duke NUS-Medical School, Singapore Eye Research Institute, Karolinska Institutet, University of Adelaide and Cambridge University, was conducted with glaucoma patients in Melbourne at private ophthalmic clinics and the Royal Victorian Eye and Ear Hospital.

For the first time, we have shown that daily high doses of vitamin B3 can lead to early and significant improvements in patients who are also receiving traditional treatments to lower eye pressure, Dr Hui said.

Glaucoma is the leading cause of irreversible blindness worldwide, characterized by gradual dysfunction and loss of retinal ganglion cells and their axons which make up the optic nerve.

The disease has a range of risk factors, including age, genetics, and elevated intraocular pressure , yet clinically available treatment strategies target only IOPlowering, and do not directly address the neurodegenerative events at the level of the retina and optic nerve.

Vitamin B3, or nicotinamide, is a precursor of NAD+ and is a critical coenzyme found in every cell in the body, involved in hundreds of metabolic processes. Recent work has indicated that glaucoma patients are low in NAD+ serum.

While retinal ganglion cells endure significant metabolic stress in glaucoma but maintain capacity to recover function, and B3 supplementation provides robust protection of retinal ganglion cells in preclinical models. However, the potential of nicotinamide in human glaucoma is unknown.

To determine whether functional improvements observed with NAM supplementation in mice could be reproduced in humans with glaucoma in an interventional study, researchers from the Center for Eye Research Australia conducted interventional research.

Researchers from the Centre for Eye Research Australia in Melbourne are conducting a world-first human trial of an over-the-counter vitamin supplement to treat glaucoma, a disease of the optic nerve which affects 60 million people worldwide.

Professor Jonathan Crowston is the lead investigator of the study and Dr Flora Hui is the Research Fellow conducting the six-month clinical trial. They hope to prove that therapeutic use of high dosage vitamin B3 could be used to support existing therapies for glaucoma, such as daily eye drops or in severe cases, surgery.

Imagine your cars engine is running a bit rough and as a result, the car doesnt drive smoothly. If you top up the engine with oil, the car runs better, even though you havent fixed the underlying problem, explains Dr Hui.

Our study hopes to confirm that vitamin B3 can protect nerve cells from dying, in a similar way that adding oil to a faulty car engine can still allow it to run more smoothly, she concludes.

Professor Jonathan Crowston leads the Glaucoma Research and is Managing Director of the Centre for Eye Research Australia. Glaucoma currently has no cure and vision loss is generally thought to be irreversible, he said.

How Far Along Is The Research

Many studies into vitamins and eye health have not yet reached the clinical trial stage of research, and even fewer have passed the stage of animal trials. Whats more, a lot of the current studies and their findings have come up with conflicting results. This does not necessarily mean that the research isnt correct, simply that the findings need more investigation to find conclusive evidence of the theories being put forward.

Antioxidant Vitamins C And E

One study suggests that antioxidants such as vitamin E could play a role in changes in the trabecular meshwork the filter through which fluid in the eye drains away, while another found that people in Japan with normal-tension glaucoma had lower serum levels of vitamin C.5,6

A third study found that the levels of vitamins C and E were significantly lower in the fluid inside the eye for people with two common types of glaucoma as compared to cataract patients.7

Daily recommendations for niacin in the diet of healthy individuals are:

    In a first-of-its-kind clinical report, retina specialists at the New York Eye and Ear Infirmary of Mount Sinai have shown that severe vision loss from a self-prescribed high dose of over-the-counter niacin is linked to injury of a specific cell type in a patients eye. The experts report that discontinuing the vitamin led to reversal of the condition and have published their findings in the fall issue of Journal of VitreoRetinal Diseases.

    Niacin, also known as vitamin B3, is used for lowering hyperlipidemia or cholesterol and comes in prescription and over-the-counter forms. It can produce a rare toxic reaction called niacin-induced cystoid maculopathy, a form of retinal swelling.
