In a model of leporine cartilage explants, investigated the effects of oxidative stress and antioxidants on cartilage matrix degradation . When the explants were treated with hydrogen peroxide and calcium ionophore, the cartilage matrix degraded, releasing proline, the major component of collagenous polypeptides, which was then detected by the researchers . Pretreatment with superoxide dismutase and catalase did not prevent the chondrocyte-mediated matrix degradation, maybe because the molecules were too large to penetrate the explants . On the other hand, vitamin E at 250 and 500 mM prevented matrix degradation more effectively than vitamin C, indicating that the process may be mediated by lipid peroxidation . This was evidenced by the observations that vitamin E also lowered malondialdehyde and hydroxynonenal-protein adducts from chondrocytes . Other inhibitors of lipid peroxidation, like butylated hydroxytoluene and deferoxamine, also prevented matrix degradation in the explants in a manner similar with vitamin E .

Serum vitamin E levels more than doubled over 12 weeks in the treatment group in a trial in RA, but Edmonds found no significant differences between groups in three clinical outcomes regarded by the authors as indicators of inflammation. There were significant differences favouring vitamin E in patient recorded morning pain, evening pain and pain after a chosen activity, but these analyses used one-tailed tests. A re-analysis confirmed significant differences favouring the vitamin E group for morning and evening pain but not for pain after a chosen activity. It is not clear how daily patient data for the pain scales were gathered and which of these data were used in the analysis at 12 weeks. Differences between groups in patient global assessments failed to reach significance while investigator assessments significantly favoured vitamin E. Even so, according to investigator global assessments, 60% of patients showed no change or got worse. Baseline data indicate that the vitamin E group had more severe RA than controls. The authors concluded that vitamin E has analgesic but not anti-inflammatory effects. The latter conclusion was supported by haematological and biochemical data gathered during the study.

The second RCT comparing vitamin E with placebo is that of Jäntti described earlier. The abstract available provides no assessable data but the authors concluded there was no clear effect of vitamin E on RA.

Trials For Rheumatoid Arthritis

Three of the four trials in this review article investigated the potential role of vitamin E in treating rheumatoid arthritis, while the fourth studied the effectiveness of selenium ACE treatment.

  • In the first trial , there was no significant difference between participants on 1,200 mg per day vitamin E supplements or a placebo in morning stiffness, number of swollen joints and degree of joint tenderness, but there was a significant difference in pain reduction in favour of participants taking vitamin E.
  • In both the second trial and the third trial , there was no significant difference between participants allocated 1,200 mg a day vitamin E supplements or participants given 150 mg diclofenac a day in all aspects of disease severity at the end of the trial.
  • In the fourth trial , there was no significant difference between participants who received selenium ACE supplements or participants on a placebo in all aspects of disease severity at the end of the trial.

A trial of low quality. Results of this trial were given a lower weighting when we came to our conclusion about the compound.
